Who is to be blamed ???

Who is to be blamed for thursdays railway accident which killed 17 people and injured 125. This news article(http://timesofindia.indiatimes.com/articleshow/1086225.cms) holds the linesman guilty, but is this fair for the lines man, is he expected to make no mistake in say 30 years of service. Yes, I know that one should be accountable for his actions and if a mistake costs a life then it cant be pardoned but asking a human not to err ever is impractical.
About 99% of the rail accidents in India are caused due to human error but still no anti collision devices are installed on the trains, if human error has caused deaths in the past then why are petty linesmen allowed to kill people? These accident could easily have been avoided had the railways showed any will to use new technology which I am sure is easily available.
In my view celebrating passenger weeks and reducing the fares are not the steps to be taken to show care for passenger, railways can win the hearts only if they take concrete steps to ensure passenger safety.
